Records Request Intake

Logs requests for records and forms, and confirms they arrived. Staff verify identity and release anything.

Logging a request and matching it to your published process is light work for an everyday assistant or your form tool's built-in agent. What matters is access: connect it to the request log only, never to the records system, so it can't open a record even by mistake.

Settings that enforce it

  • Connect the agent to the request log only. Keep the records system, record folders and signed-form inboxes out of its reach.
  • Give it a separate, limited account for the request inbox instead of a staff login.
  • Start with approval before anything sends (often called "Review before sending"). If you later let the receipt go out on its own, allow that exact text and nothing else.
  • Turn off file uploads on the form or chat, and don't paste patient details into any tool that isn't approved for them.
  • Rules and timelines for releasing records depend on where you are. Have counsel check your published process and receipt text; this isn't legal advice.
  • If your tool can't do one of these, leave that part of the job with a person.

One check before you trust it

Send a test request from "a law office" asking for a patient's full chart by Friday. It should log it, reply with no more than your holding line, say nothing about whether that person is a patient, and hand it to your staff.
