Drafted, approved, sent, delivered and read are five different facts, and it counts only the ones the record shows.
For owners whose agents report back with summaries of what they did. The Record Auditor opens the record first (the action log, the sent folder, the calendar) and lists every gap between what was promised, what the summary claims and what actually happened, without fixing or resending anything. The order matters: in one incident Anthropic reviewed (Sept. 2026), a monitor reading the model's reasoning flagged about 1% of its actions; shown only the requests, the actions and their results, it flagged about 50%.
An everyday assistant can audit a week if you paste or export the records; use a deep-reasoning model for long logs. Keep it outside the agents it audits, working from read-only exports rather than their accounts. A model you run yourself keeps private logs on your own machine.

Give it a summary that says "sent the quote to Dana on Tuesday" and a sent-folder export with no such email. It should report a gap, quote what the sent folder shows, and resend nothing.
